This form is to gather information for students in grades 9 and 10 to evaluate interest in attending face-to-face (4 days per week) or to remain remote for the remainder of the semester. Parents, if you wish for your scholar to attend four days of face to face instruction, please, fill out this survey and submit by Monday, March 29, 2021, at 5:00 pm.
